Site representative with a preprinted delivery ticket for each delivery of concrete on which is printed, stamped or written the minimum information detailed below. The delivery ticket will form the basis of invoicing. The following information shall be included in the delivery ticket to accompany the load to engg



(a) Name or number of the ready-mixed concrete depot.

 (b) Serial number of the ticket.

(c) Date.

(d) Truck number.

(e) Name of the Purchaser, Name and location of site. 

(f) Grade or mix description of the concrete.


(g) Specified target workability.


(h) Minimum cement content (if specified).

(i) Type of cement and grade (if specified).


(j) Maximum free water-cement ratio (if specified).


(k) Nominal maximum size of aggregate.


(l) Generic type or name of any chemical and mineral admixtures included. (m) Workability/Slump at plant


(n) Admixture dosage added at plant


(o) Quantity of concrete in cubic meter.


(p) Time of loading.


(q) Signature of the plant operator.
